1. Collection of Personal Information
When you visit the Site, you may be asked to provide certain information such as your name, mailing address, e-mail address, and telephone number, and for on-line purchases, your billing and shipping information, and your credit card information (“Personal Information”) for certain purposes described in this Section II. We only collect Personal Information that is voluntarily provided to us by our users. However, if you opt not to provide us with certain Personal Information through the Site, you may be unable to participate in certain promotions, receive product information, or buy products offered directly or indirectly through the Site.
The copy, claims, and offers contained in this website, which is regulated by the Utah Division of Occupational and Professional Licensing, are not intended for use by residents of foreign countries.
2. Methods of Collections and Use
We may collect and use your Personal Information in any of the following ways:
You will need to complete an electronic registration form in order to create an account with our Site (“Site Account”). During registration, you will be required to provide Personal Information, which includes your name, e-mail address, and telephone number. You will also be required to provide a password, which will be used to log-in to your Site Account. In addition, we will provide you with the ability to “opt-in” to additional communications from us, our Affiliates, and/or our third party direct marketing partners.
It is important that you protect and maintain the security of your Site Account. If you forget your password, you may not be able to access certain features or functionality of the Site, including but not limited to the ability to manage your Site Account. You may retrieve your password by clicking on the [“forgot my password”] button at the Site Account log-in screen.
In an attempt to process your order we may contact you by one of the following ways:

Notifications: a notification is an electronic message or reminder within our third party eVisit app
The Personal Information we collect when you make a purchase through the Online Store may be used by Upscript as follows: (i) for internal tracking, (ii) for order placement and shipping (including, without limitation, to provide you with automated order status updates via e-mail), (iii) for information correction purposes, (iv) for internal record keeping, (v) for determining a means to contact you to respond to your questions or requests, (vi) to provide you with information regarding changes to the Site which may affect your Site account and/or your use of the Site, and (vii) to inform you of any product or promotional offers from us, our Affiliates, or from our third party direct marketing partners.
As a customer using our site, Upscript may disclose personal and health information to Teva, Inc to communicate with you for marketing purposes.
Most of the information collected in the registration process will be used to process your order through the Online Store. During the ordering process, you will be required to provide financial information, such as your shipping and billing address, credit card number and expiration date. This information is used for billing purposes and to fulfill your order. If we have trouble processing an order, we will use your Personal Information to communicate with you. To properly process your credit card information, we must share your personal and financial information with a merchant bank for authorization and approval. This process is protected by an enhanced security system. We do not share your financial Personal Information with any third parties, except as specified herein.
For video consultation calls between you and physician, we have partnered with a third party provider of telehealth services and will be passing along relevant information so that your assigned physician has the appropriate information needed to review your medical record. WebRTc technology is used to facilitate the video consultation with a physician. Questions, answers and interactions with a physician via our third party video consultation solution are private and secure.
If you choose to reject the cookie, you may still browse our Site and our Store, but you will be unable to purchase products from us through the Store using the shopping cart functionality contained therein. Our shopping cart at www.Upscript.com cannot function without cookies enabled so that the necessary information to process your order is retained. If you disable cookies, Upscript will be unable to accept your online order.
We will not share any Personal Information provided by your cookies with any third party, though you agree and acknowledge that we may share non-personal information with third parties. We will, however, link data stored in cookies to the Personal Information you submitted while on our Site. This allows us to personalize your shopping experience and discern user Site preferences.
d. Log Files
Upscript uses the IP address from your Internet connection to analyze trends, administer the site, track users’ movements, and gather broad demographic information for aggregate use and to improve the user experience. IP addresses are not linked to any personally identifiable information.
We may conduct occasional online surveys via our website or via an email newsletter. These surveys are entirely voluntary and you may decline to participate.
3. Sharing of Personal Information with Third Parties
We may share your Personal Information with third parties in any one or more of the following ways:

You may “opt-in” and thereby give your consent to receive communications from our third party direct marketing partners. Please note that if we receive your “opt-out” request after we have already shared your Personal Information with a third party, we may not have the ability to control the use of your Personal Information by such third party. Upscript is not responsible for and disclaims all liability for any use of the information you choose to provide to such third parties.
d. Physicians or pharmacy staff
At the time you place an order for a prescription item, we are required to obtain an approval from a physician. We will need to share basic information with a physician so your medical file can be reviewed. Some states may require a video consultation to occur between you and a physician. We have partnered with a third party software company to facilitate that virtual communication. We also share information (medication ordered, shipping address, allergies, etc) with our pharmacy staff to allow them to dispense the medication. If the pharmacy has a question; they will contact you prior to dispensing your medication.
In the event of an emergency, please go to an emergency room immediately; do not use our video consultation solution.
e. Special Events
If you elect to participate in any promotions, sweepstakes, surveys, questionnaires or other events during your visit to our Site, the rules or terms and conditions of those events may indicate that your Personal Information will be shared with third parties. By choosing to participate and submitting your Personal Information with respect to such events, you consent to disclosure of your Personal Information to such third parties. Upscript is not responsible for and disclaims all liability for any use of the information you choose to provide to such third parties.
f. Fraud Protection and Compliance with Law
We may disclose any information, including Personal Information, we deem necessary, in our sole discretion, to comply with any applicable law, regulation, legal process or governmental request. We may also exchange information, including Personal Information, with other companies and organizations for credit fraud protection and risk reduction.
g. Change of Control
If we are acquired by or merged with another company, if substantially all our assets are transferred to another company, we may transfer information collected from you to that other company. .
h. Anonymous and aggregate information
We may gather and disclose information about you that is not personally identifiable. For example, we may publish reports that contain aggregate and statistical data about our customers. These reports would not contain any information that would enable the recipient to contact, locate or identify you.
III. COLLECTION OF NON-PERSONAL INFORMATION
We also collect aggregate, non-personally identifiable information about you that is transferred to us by your browser and/or by cookies when you access our Site, and you may be asked to provide non-personally identifiable information while using certain features or functionality available on the Site, such as zip code, shopping habits, and product and information preferences (collectively, “Non-Personal Information”). Like most standard website servers, we use log files to collect Non-Personal Information, including internet protocol (IP) addresses, browser types, internet service providers (ISPs), referring/exit pages, platform types, date/time stamps, and the number of clicks. Non-Personal Information collected is used to analyze trends, administer the Site, track user movement in the aggregate, and gather broad demographic information for aggregate use. We do not link IP addresses to Personal Information.
Upscript implements various security measures in accordance with industry standards to protect the security of your Personal Information and Non-Personal Information both online and offline. When you are requested to submit Personal Information (such as your credit card number) that information is encrypted using secure-socket-layer (“SSL”) technology. Our corporate values, ethical standards, policies and practices are committed to the protection of customer information. In general, our business practices limit employee access to confidential information to those employees we reasonably believe need such information to do their jobs.
All Upscript servers, including web servers and database servers are housed and maintained in secure locations. Access to the database is strictly monitored and protected from outside access. Internet access is restricted and protected by firewalls. The servers on which we store Personal Information are maintained in a secured environment, inside a secured and locked room. All backups are stored and locked in a high-level security room. Only personnel with proper security clearance have access to these restricted areas.
Notwithstanding anything contained in this Section, while we take reasonable precautions against possible security breaches of the Site and our customer databases and records, no website or Internet transmission is completely secure. Consequently, Project Ruby, LLC cannot and does not guarantee that unauthorized access, hacking, data loss, or other breaches will never occur. We urge you to take steps to keep your Personal Information safe (including your Site Account password), log out of your Site Account after use, and close your web browser.

Alternatively, you may contact us at service@Upscript.com and request to review, amend, or delete your Personal Information collected by us, and/or request us to cease collecting Personal Information from you in the future.
If you do not want to receive special offers, marketing communications, newsletters or other promotional communications (“Marketing Communications”) from: (i) Upscript (ii) our Affiliates, provides several ways for you to “opt-out” or withhold your consent from receipt of such Marketing Communications. For users who have a Site Account, you may opt-in or opt-out at the time. Alternatively, you may “opt-out” of Marketing Communications that you are already receiving from us, our Affiliates or our third party direct marketing partners by following the steps described in each Marketing Communication, and/or you may send an e-mail to service@Upscript.com with a request to stop all Marketing Communications. What are sildenafil tablets?
Sildenafil tablets are a prescription medicine used to treat erectile dysfunction (ED). You will not get an erection just by taking this medicine. Sildenafil tablets help a man with erectile dysfunction get and keep an erection only when he is sexually excited (stimulated).
Sildenafil tablets are not for use in women or children. It is not known if sildenafil tablets are safe and effective in women or children under 18 years of age.
IMPORTANT SAFETY INFORMATION
What is the most important information I should know about sildenafil tablets?
Sildenafil tablets can cause your blood pressure to drop suddenly to an unsafe level if they are taken with certain other medicines. Do not take sildenafil tablets if you take any other medicines called “nitrates.” Nitrates are used to treat chest pain (angina). A sudden drop in blood pressure can cause you to feel dizzy, faint, or have a heart attack or stroke.
Do not take sildenafil tablets if you take medicines called guanylate cyclase stimulators which include:
- Riociguat (Adempas®) a medicine that treats pulmonary arterial hypertension and chronic-thromboembolic pulmonary hypertension.
Tell all your healthcare providers that you take sildenafil tablets. If you need emergency medical care for a heart problem, it will be important for your healthcare provider to know when you last took sildenafil tablets.
Stop sexual activity and get medical help right away if you get symptoms such as chest pain, dizziness, or nausea during sex. Sexual activity can put an extra strain on your heart, especially if your heart is already weak from a heart attack or heart disease. Ask your doctor if your heart is healthy enough to handle the extra strain of having sex.
Sildenafil tablets do not protect you or your partner from getting sexually transmitted diseases, including HIV—the virus that causes AIDS.
Who should not take sildenafil tablets?
Do not take sildenafil tablets if you:
- take medicines called nitrates (such as nitroglycerin)
- use street drugs called “poppers” such as amyl nitrate or amyl nitrite, and butyl nitrate
- take any medicines called guanylate cyclase stimulators such as riociguat (Adempas)
- are allergic to sildenafil, as contained in sildenafil tablets and REVATIO®, or any of the ingredients in sildenafil tablets. See the end of the Patient Information leaflet for a complete list of ingredients in sildenafil tablets.
What should I tell my healthcare provider before taking sildenafil tablets?
Before you take sildenafil tablets, tell your healthcare provider if you:
- have or have had heart problems such as a heart attack, irregular heartbeat, angina, chest pain, narrowing of the aortic valve or heart failure
- have had heart surgery within the last 6 months
- have pulmonary hypertension
- have had a stroke
- have low blood pressure, or high blood pressure that is not controlled
- have a deformed penis shape
- have had an erection that lasted for more than 4 hours
- have problems with your blood cells such as sickle cell anemia, multiple myeloma, or leukemia
- have retinitis pigmentosa, a rare genetic (runs in families) eye disease
- have ever had severe vision loss, including an eye problem called non-arteritic anterior ischemic optic neuropathy (NAION)
- have bleeding problems
- have or have had stomach ulcers
- have liver problems
- have kidney problems or are having kidney dialysis
- have any other medical conditions
Tell your healthcare provider about all the medicines you take, including prescription and over-the-counter medicines, vitamins, and herbal supplements.
Sildenafil tablets may affect the way other medicines work, and other medicines may affect the way sildenafil tablets work causing side effects. Especially tell your healthcare provider if you take any of the following:
- medicines called nitrates (see “What is the most important information I should know about sildenafil tablets?”)
- medicines called guanylate cyclase stimulators, such as riociguat (Adempas)
- medicines called alpha blockers such as Hytrin® (terazosin HCl), Flomax® (tamsulosin HCl), Cardura® (doxazosin mesylate), Minipress® (prazosin HCl), Uroxatral® (alfuzosin HCl), Jalyn® (dutasteride and tamsulosin HCl), or Rapaflo® (silodosin). Alpha-blockers are sometimes prescribed for prostate problems or high blood pressure. In some patients, the use of sildenafil tablets with alpha-blockers can lead to a drop in blood pressure or to fainting.
- medicines called HIV protease inhibitors, such as ritonavir (Norvir®), indinavir sulfate (Crixivan®), saquinavir (Fortovase® or Invirase®) or atazanavir sulfate (Reyataz®)
- some types of oral antifungal medicines, such as ketoconazole (Nizoral®), and itraconozale (Sporanox®)
- some types of antibiotics, such as clarithromycin (Biaxin®), telithromycin (Ketek®), or erythromycin
- other medicines that treat high blood pressure
- other medicines or treatments for ED
- sildenafil tablets contain sildenafil, which is the same medicine found in another drug called REVATIO. REVATIO is used to treat a rare disease called pulmonary arterial hypertension (PAH). Sildenafil tablets should not be used with REVATIO or with other PAH treatments containing sildenafil or any other PDE5 inhibitors (such as Adcirca® [tadalafil]).
Ask your healthcare provider or pharmacist for a list of these medicines, if you are not sure. Know the medicines you take. Keep a list of them to show to your healthcare provider and pharmacist when you get a new medicine.
What are the possible side effects of sildenafil tablets?
Sildenafil tablets can cause serious side effects. Rarely reported side effects include:
- an erection that will not go away (priapism). If you have an erection that lasts more than 4 hours, get medical help right away. If it is not treated right away, priapism can permanently damage your penis.
- sudden vision loss in one or both eyes. Sudden vision loss in one or both eyes can be a sign of a serious eye problem called non-arteritic anterior ischemic optic neuropathy (NAION). It is uncertain whether PDE5 inhibitors directly cause the vision loss. Stop taking sildenafil tablets and call your healthcare provider right away if you have sudden vision loss in one or both eyes.
- sudden hearing decrease or hearing loss. Some people may also have ringing in their ears (tinnitus) or dizziness. If you have these symptoms, stop taking sildenafil tablets and contact a doctor right away.
The most common side effects of sildenafil tablets are: headache; flushing; upset stomach; abnormal vision, such as changes in color vision (such as having a blue color tinge) and blurred vision; stuffy or runny nose; back pain; muscle pain; nausea; dizziness; and rash.
In addition, heart attack, stroke, irregular heartbeats and death have happened rarely in men taking sildenafil tablets. Most, but not all, of these men had heart problems before taking sildenafil tablets. It is not known if sildenafil tablets caused these problems.
Tell your healthcare provider if you have any side effect that bothers you or does not go away. These are not all the possible side effects of sildenafil tablets.
